Want to Learn More or Ready to Enroll?

If you're interested in our district, we recommend that you schedule a tour of our schools to see all that Three Lakes School District has to offer. Simply call our business office at 715-546-3496. 

If you're moving into the district, you'll need to complete an enrollment form. To accommodate all families, we offer three ways to complete your enrollment:

1. Download the fillable enrollment form: Best for completing on a desktop or laptop computer. Once filled out, you can email it to Nikki Maney, our enrollment coordinator, at nmaney@threelakessd.k12.wi.us.
2. Complete enrollment via Google Forms: Best for smartphones or tablets, or for devices not able to open pdf documents. Submitting this form online automatically sends your information directly to our office - no scanning or emailing required.
3. Pick up or request a paper copy: You can stop by any school office to pick up a printed form, or call to have one mailed directly to your home. Completed paper forms can be dropped off or mailed back to the school at 6930 W. School St., Three Lakes, WI 54562.
