Three Lakes School District - Activities Forms

Students and parents must have forms filled out, signed and on file with the office before they can practice, compete or participate in Three Lakes activities.

A physical is required every other year to participate in athletics. So, if a student has a physical form on file in the office for the current year, only an alternate year card is required the following school year.

Athletic Physical Forms

Handbook/Parent Permission Form

This form must be filled out once each year. This form includes parent permission to participate in sports as well as a co-curricular handbook acknowledgment.
