Students and parents must have these forms filled out, signed and on file with the office before they can practice and compete in Three Lakes athletics. These forms can be printed out, signed and returned to school.
Athletic Code Worksheet
Students must either attend a meeting in the summer before the school year begins OR complete this sheet to demonstrate understanding of the athletic code.

Read and return pages 21-24, completed and signed.
WIAA Physical Form
Students must have a physical once every two years.
